Appointment Cancellations & No-Shows

Please call (502) 883-0227 at least 24 hours before your appointment if you need to cancel or reschedule. This allows us to offer the time to another patient who is waiting to be seen.

An appointment missed without notice is recorded as a no-show. Patients with repeated no-shows may be asked to schedule only as same-day or walk-in availability allows, or may be discharged from the practice.

If you are running late, please call. Arriving more than 15 minutes late may require rescheduling so that other patients are not delayed.

Prescription Refills

Request refills through your pharmacy, the Patient Portal (https://llc.myeps3.com/PP/), or by calling (502) 883-0227. Please allow 2 to 3 business days. Refills are not processed on weekends or holidays, so request them before you run out.

Certain medications require a recent office visit before they can be renewed. If it has been more than a year since your last visit, or if your doctor needs to re-evaluate your treatment, we will ask you to schedule an appointment.

Controlled substances are prescribed only after evaluation by one of our physicians and in accordance with Kentucky law. Early refills are not provided for lost or stolen medications.

Test Results

Results are released to the Patient Portal after your doctor has reviewed them. If a result requires a change in treatment, our office will call you. If you have not heard from us within 7 to 10 days of a test, please call (502) 883-0227.

Results are not given out by email. For your privacy, we release results only to the patient or to a person the patient has authorized in writing.

Medical Records

To request a copy of your records, complete a records release form at our office or call (502) 883-0227 to have one sent to you. Please allow up to 30 days for processing, as permitted by law; most requests are completed much sooner.

Other providers may fax records requests to (502) 410-0484. Records sent directly to another treating provider for continuity of care are provided at no charge.

Referrals

If your insurance requires a referral to see a specialist, it is your responsibility to obtain it from your primary care provider before your visit. Visits without a required referral may not be covered by your insurance and may need to be rescheduled.

Payment

Co-pays, deductibles, and co-insurance are due at the time of service. We accept cash, check, Visa, MasterCard, and Discover.

Patients without insurance, or who choose not to use it, receive a discounted self-pay rate. Self-pay balances are due in full at the visit.

Communication & Privacy

Email (louisville.lung@gmail.com) is for general, non-urgent questions only. Do not email medical questions, symptoms, or requests for refills; use the phone or Patient Portal instead.

Phone calls to our office may be recorded and may be answered or assisted by an automated assistant for scheduling and general questions. Clinical questions are always directed to our staff and physicians.

We follow HIPAA privacy rules. Our Notice of Privacy Practices is available at check-in and on our New Patients page.

Emergencies

Our office is not an emergency facility. If you have severe shortness of breath, chest pain, coughing up blood, blue lips, or confusion, call 911 or go to the nearest emergency room immediately.
