What are your office hours?
We are open Monday through Friday, 8:30 AM to 4:30 PM. Closed Saturday and Sunday. We are closed on major holidays.
Where are you located?
Our office is at 1015 Dupont Rd, Louisville, KY 40207, in the St. Matthews area of Louisville. Free parking is available at the building.

What should I do if I have an emergency?
If you are having severe shortness of breath, chest pain, coughing up blood, or any medical emergency, call 911 or go to the nearest emergency room immediately. Do not wait for a call back from our office.
What if I need help after hours or on the weekend?
Our office is closed evenings and weekends. For urgent, non-emergency concerns, call (502) 883-0227 and follow the after-hours instructions. For emergencies, call 911.
Do you offer telehealth or video visits?
Telehealth availability depends on the type of visit and your insurance. Please call (502) 883-0227 to ask whether your appointment can be done by video.
Do you provide interpreter services?
Please let us know when scheduling if you need an interpreter so we can make arrangements before your visit.

Appointments & Scheduling

How do I schedule an appointment?
Call us at (502) 883-0227 during office hours, or submit a request online at louisvillelungcarepllc.com/schedule and our staff will call you back to confirm a date and time.
Do I need a referral to be seen?
Many insurance plans (especially HMO and Medicaid plans) require a referral from your primary care provider before you can see a specialist. Please check with your insurance. If you already have a referral, ask your doctor's office to fax it to (502) 410-0484. Our team can help coordinate with your PCP.
Are you accepting new patients?
Yes. All three of our pulmonologists are accepting new patients.
Can I request a specific doctor?
Yes. When you schedule, you may ask for Dr. Moutaz Al Nabhan, Dr. Ehab Haj Ali, or Dr. Sajjad Jameel, or choose the first available provider.
How do I cancel or reschedule an appointment?
Call (502) 883-0227 as early as possible, ideally at least 24 hours before your appointment, so we can offer the time to another patient.
What happens if I miss an appointment?
Please call us as soon as you can to reschedule. Repeated missed appointments without notice may affect your ability to schedule future visits. See our Office Policies page for details.
How early should I arrive?
New patients should arrive 15 to 20 minutes early to complete paperwork. Returning patients should arrive 10 minutes early.
How long will my visit take?
New patient visits typically take about 45 to 60 minutes. Follow-up visits are usually 15 to 30 minutes. Visits that include testing may take longer.

New Patients

What should I bring to my first visit?
Bring your photo ID, insurance card, a list of all current medications and dosages (including inhalers), any recent chest X-ray or CT scan reports or a CD of the images, and your referral authorization if your insurance requires one.
Can I fill out paperwork ahead of time?
Yes. New Patient Registration, Medical History, and HIPAA forms are available to download at louisvillelungcarepllc.com/new-patients. Completing them in advance saves time at check-in.
I was seen by one of your doctors in the hospital. Am I a new patient?
If you have never been seen in our office (clinic), you are considered a new office patient even if one of our doctors treated you in the hospital. Please schedule as a new patient.

Prescriptions & Refills

How do I request a prescription refill?
The fastest way is to ask your pharmacy to send a refill request to our office, or submit the request through the Patient Portal at https://llc.myeps3.com/PP/. You may also call (502) 883-0227. Please have the medication name and pharmacy ready.
How long do refills take?
Please allow 2 to 3 business days for refill requests. Request refills before you run out. Refills are not processed on weekends or holidays.
Can I get a refill if I haven't been seen in a while?
Some medications require a recent office visit before they can be refilled. If it has been a long time since your last visit, we may ask you to schedule an appointment.

Test Results & Medical Records

How will I get my test results?
Results are posted to the Patient Portal (https://llc.myeps3.com/PP/) once reviewed by your doctor. Our office will also call you if your results need follow-up. If you have not heard from us within 7 to 10 days after a test, call (502) 883-0227.
How do I sign up for the Patient Portal?
Go to https://llc.myeps3.com/PP/. Returning patients click "Existing Patient." Only register as a "New Patient" if you have never been seen in our clinic. For identity verification, the last four digits of your Social Security number must be on file with our office.
How do I request a copy of my medical records?
Call (502) 883-0227 or stop by the office to complete a records release form. We can send records to you or directly to another provider. Records can also be faxed to (502) 410-0484 by other offices.

Services & Procedures

What conditions do you treat?
We diagnose and treat asthma, COPD, chronic bronchitis, emphysema, pneumonia, pulmonary fibrosis and other interstitial lung diseases, chronic cough, shortness of breath, lung nodules, obstructive sleep apnea, insomnia and other sleep disorders, and complex or critical respiratory illness.
What tests and procedures do you offer?
Pulmonary function testing (breathing tests), sleep studies (polysomnography), bronchoscopy, endobronchial ultrasound (EBUS), and chest X-ray and CT interpretation.
What is a sleep study and where is it done?
A sleep study records your breathing, oxygen level, and sleep patterns overnight to check for sleep apnea and other sleep disorders. Depending on your situation, it may be done at home with a portable device or overnight in a sleep lab. Your doctor will explain which type is right for you, and our staff will schedule it. What is a pulmonary function test (PFT)?
A PFT is a set of breathing tests done in the office. You breathe into a mouthpiece connected to a machine that measures how much air you can move and how well your lungs work. It takes about 30 to 45 minutes and is painless.
What is a bronchoscopy?
A bronchoscopy lets the doctor look inside your airways with a thin, flexible camera and, if needed, take samples. It is done in the hospital with sedation. You will receive specific preparation instructions before the procedure.
Do you treat sleep apnea and manage CPAP?
Yes. We diagnose sleep apnea, order sleep studies, prescribe CPAP and other therapies, and provide ongoing follow-up. Bring your CPAP data card or device information to follow-up visits if you have one.

Insurance & Billing

What insurance do you accept?
Commercial plans: Anthem Blue Cross Blue Shield, Humana, UnitedHealthcare, Aetna, Cigna, Medical Mutual, and MultiPlan. Medicare and Medicaid: Traditional Medicare, Medicare Advantage plans, Kentucky Medicaid, Passport Health Plan, Humana Healthy Horizons, WellCare of Kentucky, and Anthem Medicaid. Military: Tricare, ChampVA, and Veterans Affairs with authorization. If your plan is not listed, call (502) 883-0227 to verify.
What if I don't have insurance?
We offer a discounted self-pay rate for patients without insurance. Payment is due in full at the time of the visit.
When are co-pays due, and what payment methods do you accept?
Co-pays and deductibles are due at the time of service. We accept cash, check, Visa, MasterCard, and Discover.

Who are your doctors?
Dr. Moutaz Al Nabhan (pulmonary, critical care, and sleep medicine), Dr. Ehab Haj Ali (pulmonary and critical care), and Dr. Sajjad Jameel (pulmonary, critical care, and sleep medicine). All are board-certified pulmonologists with fellowship training from the University of Louisville.

Our physicians care for patients at Norton Healthcare hospitals, Kindred Healthcare, Jewish Hospital, and St. Mary & Elizabeth Hospital.

How do I refer a patient?
Fax the patient's demographics, insurance information, and recent clinical notes to (502) 410-0484. We will contact the patient directly to schedule once the referral is received.
